MFHS Circle of Care – A One Stop Shop Model

The Circle of Care is a resource for low-income or uninsured women to gain access to health and nutrition programs that prevent disease, promote wellness and encourage healthy lifestyles. For the past 14 years, this coordinated and highly effective one-stop-shop service model has represented a true “circle” for women’s health care. It fills a critical need for low-income women in Lackawanna County, Pennsylvania.

  • Cross referral between programs
  • Early enrollment in WIC for prenatal women
  • A continuum of care for women and children with wrap around services
  • Improved access to a variety of health and nutrition services
  • Better customer service to WIC participants
