A WIC Center located in a Primary Care provider Setting
In 2013, Maternal and Family Health Services re-located a WIC Nutrition Center into the Wright Center Community Health Hub—a Level 3 Patient Centered Medical Home—improving fiscal efficiency and better customer service to our WIC Participants.
The WIC Center re-location is an example of service integration and coordination with other programs that will improve access and referral to health care services while promoting the health and well-bring of the women and children we serve through the WIC Nutrition Program.

- Formula prescriptions
- Strengthen relationship between WIC nutritionists and primary care provider for more comprehensive service to the participant
- Improved cross referrals between WIC and the primary care provider
